Retain original images and metadata under project rules. A filename or caption alone does not establish authenticity.

Caption worksheet

Describe observable facts; do not infer causes, acceptance or hidden conditions from a photograph.

  • Photo ID / record author
  • What is directly visible
  • Context reported by another person / source
  • Uncertainty or limits of the image

Evidence handoff

Follow employer and project retention and privacy requirements. Do not place confidential records in a public download.

  • Project / package reference / sender / recipient
  • Included file IDs and versions
  • Transfer method / access check / acknowledgement
  • Missing records / restrictions / follow-up owner

Open evidence questions

Close a question only when the response and evidence are recorded. This is an organization aid, not a formal chain-of-custody system.

  • Response received / date / source / closure evidence

A worked example

Fictional example: Photo P-018 shows standing water beside an access path at 10:15 a.m. The caption records the project location and viewing direction. It does not claim the source of the water or that the condition was accepted. Question E-03 asks the assigned representative for the relevant follow-up record; it stays open until a sourced response is received.
